Application requirements
Generic to all applicants:
- Completed application form – New Registration Form or Renewal Form
- Completed Statement of Experience OR Officiating/Contest Record
- Registration Fee
- 2 Passport size photographs (hard copies only)
- Proof of Age – photocopy of Drivers License or Passport
